Caída de instancia

Hola foro!

Me ha sucedido algo a lo que no le encuentro la punta del ovillo. Se calló una instancia(*), el mensaje que quedo en el online.log fue

Fatal error in ADM VP at mt.c:12516
Unexpected virtual processor termination, pid = 295108, exit = 0x90009

y con un onstat - la respuesta era

Hubo que bajar los segmentos con ipcr ya que no bajaba bien con el onmode -ky o mejor dicho no levantaba porque habian quedado estos segmentos levantados, una vez eliminados la instancia levanto sin problemas.

Revise y no hay archivos attemp generados, lo único que veo es que luego de levantarse la instancia a las 00:51 apareció este mensaje

y a las 05 se clavó...

Alguna sugerencia por donde buscar las razones de esta caida para evitar una nueva caida?
